Statement on Assistance - Word doc |

| |
Each student submitting a paper must submit the Statement on Assistance through his/her mentor. The student shall complete the form and forward to his/her mentor for approval. The mentor shall submit the form electronically to jwetstein@cos.siu.edu with a statement that indicates he/she has reviewed the form and approves of the contents. This will provide verification in lieu of a signature. |
